The Role – Project Worker (Mental Health Scheme and Dispersed / Floating Support)

Working across our Schemes in the Conwy County area, you’ll be responsible for providing an effective and efficient housing management and support service, supporting service users with improving their mental health and housing situation, drawing up and regularly reviewing individual support plans, and ensuring progress towards positive outcomes. Your duties will include:

  • Supporting service users with their mental health and wellbeing, through encouraging healthy activities and lifestyles, and signposting to services
  • Managing referrals to the scheme, contacting and interviewing applicants / agencies, arranging panel meetings, and updating colleagues within the scheme of new referrals
  • Drawing up and regularly reviewing individual risk assessments
  • Assisting service users with plans to move on to more permanent accommodation
  • Maintaining accurate service user records and files
  • Organising and managing a range of engaging activities to promote social inclusion
  • Playing a hands-on role in responding to incidents, completing room checks and ensuring properties are maintained

The Package

  • Salary of £21,205
  • Permanent contract, working 35 hours per week, Monday to Friday 9am to 5pm (some flexibility with start and finish times)

You will also have an opportunity to gain a qualification in health and social care and/or mental health whilst undertaking this role.

Our Requirements – Project Worker (Mental Health Scheme and Dispersed / Floating Support)

  • Interest in and a commitment to supporting people with their housing needs and life skills in a person-centred, trauma informed way, with a willingness to learn
  • An ability to be courteous and be able to pronounce place names accurately in Welsh
  • Good working knowledge of Microsoft Word, Excel and Outlook
  • Good verbal and written communication skills
  • A valid UK driving licence and vehicle available for work purposes
