The Role – Project Worker (Mental Health Scheme and Dispersed / Floating Support)
Working across our Schemes in the Conwy County area, you’ll be responsible for providing an effective and efficient housing management and support service, supporting service users with improving their mental health and housing situation, drawing up and regularly reviewing individual support plans, and ensuring progress towards positive outcomes.
