Liverpool Community Mental Health Team is seeking a qualified Social Worker to join our team on a 6-month contract to cover a colleague who is pursuing grant-funded activity.

Job Title: Social Worker

Contract Length: 6 months

Pay Rate: 35 per hour

Hours: Full-time

About the Role:

The successful candidate will be responsible for undertaking assessments under the Care Act 2014 and Mental Capacity Act 2005, as well as developing and implementing support plans for individuals experiencing severe and enduring mental ill health. You will work collaboratively with other professionals to ensure that individuals receive the support they need to live fulfilling lives.

Key Responsibilities:

  • Undertake comprehensive assessments of individuals' needs, including mental health, physical health, and social circumstances.
  • Develop and implement person-centered support plans that promote independence and well-being.
  • Work with individuals and their families to identify and address barriers to recovery.
  • Liaise with other professionals, such as GPs, psychiatrists, and occupational therapists, to coordinate care.
  • Promote a strengths-based approach to support planning and delivery.

Essential Requirements:

  • Degree in Social Work
  • Registration with Social Work England
  • Experience working with adults with mental health conditions
  • Knowledge of the Care Act 2014 and Mental Capacity Act 2005
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team

Desirable Requirements:

  • Experience working in a community mental health team
  • Knowledge of Liverpool City Region
